Who Should Complete This Form

This form is for a parent or legal guardian, at least 18 years old, of a young person under 18 who is taking part in a Group’s fundraising through Cold Streets.

Please do not complete it for someone else’s child. Young people should not complete this form themselves.

About This Form

Cold Streets is operated by Tammy Dennis, an individual doing business as [Cold Streets Clothing Company] (“Cold Streets”).

We work only with adults, and the Group’s approved Adult Representative manages the Group’s fundraising.

This form lets you decide whether your child’s name or image may be shown in connection with the Group’s fundraising. You may say no to everything, and your child may still take part without their name or image being shown.

Information We Ask For

Your full name, email address, relationship to the child (parent or legal guardian), the Group’s name, and, if you choose, your child’s first name or initial.

We do not ask for a child’s last name, date of birth, address, school, phone number, or email.

Your Choices

Check all that you allow. If you check the final option, it overrides the other choices.

Where It May Appear

If you give permission, the name or image may appear on ColdStreets.com, including the Fundraisers Board, state and country pages, and the Pass the Love Wall; on Cold Streets social media accounts; in emails; and in promotional materials for the Group’s fundraising.

What We Will Never Show

Even with your permission, we will not show a child’s last name, school, home address, location, phone number, email address, social media handle, or schedule.

How Long It Lasts

Your permission lasts [until you withdraw it].

No Payment, No Requirement

Your child will not be paid for any use of their name or image. Your child may take part in the Group’s fundraising whether or not you give permission.

Withdrawing Permission

You may withdraw permission at any time by emailing support@coldstreets.com with “Withdraw Consent” in the subject line and the Group’s name.

We will remove the name or image from the Site and our accounts within [10 business days].

We cannot take back copies that others have already saved, shared, or printed.

Your Confirmations

By submitting this form, you confirm that:

• You are the child’s parent or legal guardian with the legal authority to give this permission;
• The information you gave is accurate;
• You have read our Youth Safety Policy and Privacy Policy; and
• You understand that anything shown online can be copied, shared, or saved by others.

How We Use This Form

We use your information only to record your choices, contact you about this form, and show or remove the name or image as you direct.

It is collected through our form provider, Formspree, and handled as described in our Privacy Policy.

We keep this record for as long as the name or image is used, plus [X years].
